Person-Centered Plan

Together we'll collaborate to discuss your hopes and dreams. Your circle of support will also be part of this process. 

Person-Centered Plan Creation

Your Person-Centered Plan will include all the information shared in our collaborative discussion. 

Budget & Spending Plan Creation

We help you create the spending plan identifying how your budget funds will be used.

Ongoing Support

We are here to support after transitioning to the Self-Determination Program. Supports include annual renewal, spending plan revisions, and more. 

Self-Determination Program

The Self-Determination Program(SDP) is another way to receive regional center services. SDP allows you to choose how you spend your regional center budget. An Independent Facilitator guides you through the transition into the program. Start by completing the DDS orientation.
